Please make sure to take a copy of your family member’s test results to your healthcare provider, along with this letter. Your provider may order your genetic test online by following five simple steps. If your provider needs any assistance, they can call us at 800-436-3037.
To be eligible for Invitae's family testing program, your provider must place your order within 90 days of the original patient’s test report date and must have your relative’s order number “RQ number” on hand when ordering. Refer to our Family Variant Testing page for more details.
